Coastal Pediatric Associates is the first pediatric practice in the Lowcountry to receive the honor of being a designated Level 3 Patient Centered Medical Home. This means that we are recognized for providing the highest level of care in the three-tiered quality grading system that consists of being accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective.

    As your Medical Home, we will:
  • Learn about you, your family, life situation, and health goals and preferences.
  • Take care of any short-term illness, long-term chronic disease, and your all-around well-being.
  • Maintain your vaccination schedule and other preventative services.
  • Connect and coordinate your care with specialists, and other healthcare services as your health needs change.
  • Provide extended access through weekend office hours as well as early morning and evening hours.
  • Notify you promptly of test results.
  • Communicate with you and assist in helping you understand your options.
  • Help you make the best decisions for your care.
  • Give you information about classes, support groups, or other services that can help you learn more about your health and to stay healthy.
    As our patient, we hope that you will:
  • Know that you and your child are a full partner with us in your care.
  • Come to each visit with any updates on medications, dietary supplements, or remedies your child is using, and with any questions you may have.
  • Keep scheduled appointments or call to reschedule or cancel as early as possible.
  • Understand your child's health and what you can do to help them stay as healthy.
  • Work with us to develop and follow a plan that is best for your child's health, if you have obstacles to this plan please do not hesitate to discuss.
  • Ensure your child takes any medication as prescribed and update us on any new medications or changes in health status.
  • If possible, contact our triage department before going to the emergency room or urgent care so we may advise them of your medical history or provide you with a same day appointment in our office.
  • Agree that all health care providers in your care team will receive all information related to your healthcare.
  • Learn about your health insurance coverage and your benefits from your insurer.
  • Give us feedback to help us improve our care for you.
