  • Come to each visit with any updates on medications, dietary supplements, or remedies your child is using, and with any questions you may have.

  • Keep scheduled appointments or call to reschedule or cancel as early as possible.

  • Work with us to develop and follow a plan that is best for your child's health; if you have obstacles to this plan please do not hesitate to discuss.

  • Ensure your child takes any medication as prescribed and update us on any new medications or changes in health status.

  • If possible, contact our triage department before going to the emergency room or urgent care so we may advise them of your medical history or provide you with a same-day appointment in our office.

  • Agree that all healthcare providers in your care team will receive all information related to your healthcare.
